The best imaging modalities for diagnosing muscle injuries are MRI and ultrasound. Ultrasound imaging requires an experienced examiner, and the findings are depicted less clearly and objectively than by MRI. For this reason, MRI has become the imaging modality of choice for the differentiation of muscle injuries. The most common muscle injury is muscle strain (1). It is an injury to the musculotendinous junction. Typical for muscle strain is edema centered along the musculotendinous junction (1). More severe muscle strains contain fluid collections such as hematomas and may contain grossly interrupted muscle fibers and thus may show masslike features on MR images in addition to muscle edema. Typical features of muscle injuries on MRI include fluid signal intensity tracking and surrounding the muscle fibers, myofascial, myotendinous or tendinous units of the rectus femoris muscle and/or discontinuities of the respective muscle components.
